The new Waivers deputy has come up with a smart new web form for submitting event and demo waivers.
You will need to log in with your @midrealm.org address but you can fill out all the required information, take a picture (or scan) your waiver page and upload it

Per kingdom and corpora law each non-member who attends an event, or participates in activities at a demo must sign a consent to participate and hold harmless waiver. See here for the waiver rules.

Waivers must be uploaded within 30 days of the event and will be maintained for 7 years (adult) and 20 years (minors).
